Senior Care Referral Service

Senior Care Referral Service

Concierge Care Advisors is a senior care referral service with the leading experts in senior housing and elder care.

Our services are entirely free to the seniors and their families. They are compensated by the senior housing communities they refer the elders to. That being said, our Care Advisors truly have the seniors' best interests at heart and are rarely compensated in the case of those with low-income or on Medicaid. This is never an issue because our goal is to raise the bar for senior housing and elder care

Whether you or your loved one requires a 24-hour nursing home or a few hours of personal assistance each week, Concierge Care Advisors is here to be your advocate when choosing a senior living option. Our recommendations will only include the best, most qualified Providers and Professional Services. For our clients, we are a trusted partner in the process of selecting life's transition options.

Unlike other senior referral agencies, we know, visit and prequalify every senior care professional & residential community we recommend. With Concierge Care, you can expect a comprehensive and knowledgeable team approach to senior living options (assisted living, retirement communities, adult family homes, adult day care, nursing home services, Alzheimer's care), as well as ancillary services such as financial, legal and benefit planning. We do not promote one ancillary professional over another, but we do perform an extensive background check and due diligence processes on those we recommend to meet your needs.

Concierge Care Advisors eliminates the anxiety that can be associated with life’s transitions.  Whether you or a family member are at home, in the hospital, or even in a community or home care location now, we will always come to you.  And there is never a fee for using our senior care referral service; our personalized assessment of your needs, pre-arranged facility tours, and our final recommendations are all free.

Minor- to Moderate-Care Options

In-Home Care:

If staying at home is the best option for your loved one, then Concierge Care Advisors can help identify the best choices for residential care or home health care. We have met with a number of professionals who offer their services in home in the following states: Washington, California, Oregon, and Idaho. Learn more >>

Independent Living:

For some seniors, they may only need minimal assistance with housework and their yard, but more than anything, want to be in a community of their peers. These homes offer little to no additional services and are typically senior communities (aged 55 and over), but they do require much less maintenance and if your elder needs it, you can hire in-home care for these residences. Assisted Living:

These are facilities offer more support with seniors who need help with the activities of daily living (preparing meals, travelling to the grocery store, showering, etc.). These are typically apartment complexes or group-home settings, providing senior care and a community of peers. In addition, our senior care referral service only associates with those assisted living communities that have a nurse on-call 24/7. Learn more >>

High Level of Care Options

Adult Family Homes:

These homes are numerous and the level of elder care provided can range from moderate to high. These are residential homes licensed by the government and run by registered gerontologists or nurses. Adult Family Homes are actual home settings with a limit of six residents to each house. This is appealing for many families because it ensures attentive and customized care. Learn more >>

Nursing Homes:

Nursing homes are often thought of as a last resort due their ill repute in the past, however all of the facilities we affiliate with have been trying to change this image. As a senior care referral service, typically we will only recommend nursing homes if your senior needs 24-hour care and supervision, whether it’s due to a disability, memory loss, or other debilitating condition. These should not be considered end-of-life or hospice care, but they do provide the maximum level of care an elder can receive short of being in the hospital. Learn more >>

Memory Care:

Memory care is a specific form of senior care that is in addition to preexisting housing establishments. In other words, select Adult Family Homes, Assisted Living Facilities, and others provide certified memory care for seniors struggling with memory loss due to Dementia or Alzheimer’s Disease. If your loved one is in need of this kind of care, it’s imperative to contact our Care Advisors, since they know the reputable senior housing options that provide this type of care. Learn more >>

We can help you find the most suitable – and affordable – option to meet your needs, preferences, and unique situation. Each type of senior living community or facility has a unique purpose and approach. Identifying the best one can be overwhelming, that’s why our senior care referral service will help you in this challenging time.

Your Personalized Tour

After discussing the senior care options that best fit your needs, your Concierge will schedule a tour of the residential communities or facilities you’d like to inspect.  We’ll be on site with you and your family to ensure that the important questions are asked and answered. After visiting, we can help you make your final decision, whether you are selecting from basic senior living services or more specialized nursing home services.  Meeting your elder care needs is our primary goal.
